Please bring an ID, insurance card(s), and a list of current medications.

Please note that a credit card on file is required at time of check in. The card information will be securely stored and will only be charged in case of "no show" fees or balances past due. You will be notified prior to any charges placed.

Dress in loose fitting clothing so that it is easy to disrobe for your appointment.

Avoid heavy makeup.

If you are being seen for acne, rash, or eczema, please bring a list of any topical products including prescription creams, over the counter remedies, lotions, ointments, shampoos, etc.

If you are seeking a second opinion, please bring any relevant medical records (labs, biopsy results, prior medications used to treat skin condition, office visit notes).

We are located in Kulamalu Town Center off of Kula Hwy in Upcountry Maui.
We are NOT located in Makawao Town.
Starting on Kula Hwy, turn right at the stoplight onto A’apueo Pkwy (Kulamalu).
Turn left on Kupaoa St.
Turn right into the Kulamalu Plaza parking lot.
Take the elevator located between Marlow Restaurant and Mahalo AleWorks to the 2nd floor.
Turn right in the hallway towards Suite 208.

As a courtesy to our staff and other patients, please notify us in advance of any cancellations or schedule changes.
In the event that you need to cancel or reschedule your appointment, please call, text, or e-mail 24 hours prior or a no-show fee of $50 will be automatically charged to your credit card on file. If you consistently “no show” your appointments, you may not be rescheduled.
INSURANCE:
It is the patient’s responsibility to understand insurance coverage and requirements.
If your insurance is accepted by our practice, we will bill the insurance company. Keep in mind, many procedures or visits are considered “cosmetic” by insurance companies and you may need to pay out of pocket for these visits (such as skin tag removals, destruction of benign growths, milia removal, injection of acne cysts, hair loss, etc). Any charges deemed “non-covered” by your insurance company will be your responsibility and due at the time of visit.
REFERRALS:
Certain plans require referrals from your primary care physician in advance of your appointment. Usually HMO plans, Quest, and VA plans require a referral or prior authorization for specialist office visit. It is your responsibility to know if you need a referral, when that referral expires, and how many visits have been authorized. If a referral is required but not obtained at the time of visit, we will need to reschedule the appointment.
