LEVEL 1 VISIT: $75
*ESTABLISHED PATIENTS ONLY
Examples:
Follow up of single skin lesion (mole, cyst, growth, scar, wart, etc)
Or
Refill of medication for previously diagnosed stable condition (acne, eczema, psoriasis)
Or
Follow up visits at discretion of the physician
LEVEL 2 VISIT: $100
Examples:
Spot check of single skin lesion (New patient)
Or
Follow up visits at discretion of the physician
LEVEL 3 VISIT: $175
Examples:
Multiple skin lesions of concern or Full Body Skin Examination
Or
Medical dermatology consultation (acne, eczema, multiple warts, psoriasis, rash, dandruff, skin allergy)
*Additional fees may apply for any procedures
*Outside laboratory fees may apply depending on insurance coverage
SURGERY
Surgical pricing is based on time, complexity, and materials. Surgery fee will be quoted at the time of visit. Please note outside pathology lab may bill you or your insurance company for the processing of any tissues specimens.

Mohs surgery referrals may be sent to our preferred providers on Maui or to the physician of your choice.
Self pay fee schedule applies only to patients without insurance or paying out of pocket for an out of network service. Patients with contracted health insurance plans will be billed according to their assigned plan contracts.
